We encourage you to download the New Patient Forms below that pertain to you and fill them out before your appointment.
Patient Forms
Upon entering our office, our receptionist will welcome you as a member of our family. We will request that you complete our patient forms located in the New Patient Center. This paperwork provides us with your health history and information on your condition.

Consultation
Next, you will have a consultation with Dr. Taylor to discuss your health-related problems, concerns, and potential treatment options. This initial visit is designed for Dr. Taylor to learn more about you, your condition, and expectations to determine how chiropractic care can meet your goals.

Examination
After your consultation, Dr. Taylor will perform a complete chiropractic examination testing your reflexes and flexibility. Other standard range of motion, neurological, orthopedic, postural, and physical tests will be performed as well. However, nothing will be done in our office without your consent. X-rays may be recommended if Dr. Taylor feels they are necessary to treat your condition.
After a thorough consultation and exam, Dr. Taylor will carefully review your information, and present you with his recommendations at your next visit. Dr. Taylor understands that working with the spine is serious business, which is why he sets aside time to review each case before treatment starts.

X-Ray Studies
Spinal X-rays may be taken depending on your age, and situation. There are many reasons why we would not take x-rays on certain patients including pregnancy, most children, recent usable spinal x-rays and some other various situations. Aside from these situations, x-rays are a common part of the examination process when looking for vertebral subluxations. x-rays give us important information including the position of the spinal bones, and the quality of the spine and safety of adjusting it. X-rays also tell us the relative length of time subluxations may have been involved in your spine. Commonly, most patients receive two spinal x-rays at the beginning of care.
Report of Findings
Once the information is collected and examinations are performed, Dr. Taylor will give you a detailed report of all findings and answer any questions including:
- Can you help me?

- What's wrong with me?
- How long will it take to get well?
- How much will it cost?
After reviewing your health history, goals, and examining your spine and X-rays, Dr. Taylor will discuss recommendations and notify you if your condition requires care with other providers. Dr. Taylor will provide the best program for your needs.

Treatment
At the conclusion of the Report of Finings, you can choose to experience your first treatment. This may include spinal adjustments, extremity adjustment and/or various therapies to get you well!.

Wellness Program
Prior to leaving, the doctor will suggest a wellness program to incorporate outside of treatment. If you are in pain when you first come into our office this may include: ice or heat application instructions, certain activities or positions to avoid, and at home exercises and/or stretches. If you desire, our wellness team will work with you also to create healthy habits and routines for your lifestyle. Every person is unique, therefore everyone requires a customized wellness plan. The purpose of our wellness program is for you to achieve good spinal alignment, have a healthy diet, exercise, and maintain a positive mental state.
